This form is to be completed in full by a parent or carer only, and covers all regular attendance at St Mary's Junior Church on Sundays from September 2022 - August 2023. If you are a member and log-in, some of your information will have been pre populated.

Joining or re-joining Junior Church

We would like to welcome all children from birth to year 6 of primary school to join us. We need you to register every year, so please complete a new form to let is know that you will be joining us. You can complete this as early as possible which helps us to plan ahead for September. (If you have an older child - please head over to our Youth Fuel page).
